for your child, please call the provider's office to make sure that they are taking new patients.
Then, call customer service at the number listed below to change your PCP and receive a new ID card.
Primary Care Provider (PCP): A family medicine,
pediatric, internist or general practice health care provider who is responsible for providing the Primary
Care Services to CHP+ and CHP+ Prenatal Care Program members.
Specialty Care Provider: A licensed, certified or
registered health care provider who practices in a specialty care area of medicine (not family medicine,
pediatric, general practice or internist), and who is responsible for providing health care services to a
CHP+ member upon the referral of the members Primary Care provider or another CHP+ participating provider.
Ancillary Services: A Covered Service which supports the
diagnosis and/or treatment of a Member which is ordered by a Primary Care Provider or Specialty Care
Provider and which is delivered to the Member by another Participating Provider or an outpatient or home
care basis. Examples of Ancillary Services are certain pharmaceuticals, medical supplies, laboratory tests,
diagnostic or therapeutic radiology procedures, durable medical equipment and developmental evaluations as
described in the Member Benefit Booklet.
Hospital/Facilities: A health institution offering
facilities, beds, and continuous services 24 hours a day, seven days a week.
